Information submitted through forms

The feedback form requests a rating and optional comments. A name and contact information are optional. Selecting the contact-request checkbox lets staff know a response is requested. Feedback may be submitted without providing a name or contact information; technical security or hosting data may still be processed.

A contact form, when enabled, handles the name, contact details, and message entered by the visitor. These forms are for general feedback or administrative questions. Please do not include medical details, diagnoses, or personal health information. For care questions, call us.

Purpose and processing services

When form delivery is enabled, submissions are emailed to management@bridgeofcarellc.com through Google Workspace for review by authorized staff and follow-up when requested. A form displayed as unavailable cannot deliver a submission. Email delivery creates copies in the receiving mailbox. The privacy contact can answer questions about retained messages and requests concerning them.

InMotion Hosting hosts the website. When configured and enabled, Google reCAPTCHA helps prevent automated form abuse and may process technical information for that purpose. reCAPTCHA is not loaded by the feedback feature while that feature is unavailable. Google's Privacy Policy (https://policies.google.com/privacy) and Terms of Service (https://policies.google.com/terms) apply to that service. Questions about these services, retained information, or a particular submission may be directed to the privacy contact.

Technical data, cookies, and retention

Hosting and security systems may process information such as IP addresses, request times, browser information, and requested URLs. The form application does not save message contents to an application database or form-content log on the web server. Email delivery still creates a copy in the receiving email system; technical abuse-prevention records may be processed separately.

The site may use cookies or similar browser functions needed for its features or security. A browser can block or remove cookies, although doing so may affect a feature. Links or embedded resources can involve another provider's data practices. Ask the privacy contact about current website services, cookies, analytics, technical records, and retention.
